Each can work on its own, or in powerful combination. One big advantage: They can help you meet some of the Member Connection standards under NCQA's Quality Plus Program.

• Health Risk Assessments (HRA)—fully integrated with the StayWell Solutions Online™ health portal, this HRA delivers personalized content to participants based on their self-reported health status and readiness to change. Plus, you can get access to aggregate reports and participant health data in order to profile and stratify the population for health management programs and interventions.
• Personalized Health Records (PHR)—enable individuals to compile and maintain their personal health information in a secure, central location. They also can store data loaded from third-party sources, such as lab results and prescription refills.
• Online Health Improvement Programs—a module for at-risk participants that provides essential support for behavior change. Action plans include important dates, reinforcement messages, and reminders. This flexible tool allows an organization to modify these actions and steps to fit its own program requirements. Standard components of the module include: diabetes, exercise/activity, heart health, nutrition, and stress.
• Personal Messaging System—a push-messaging module that triggers secured messages to participants based on events that take place within the tools described above, such as the HRA and upload of personal data into the Personal Health Record. Select from our message database or create your own.
• Health Coach Portal—a cost-effective platform that helps health coaches deliver personalized education and support to guide participants toward positive health behavior change and useful wellness information and programs. Health coaches using this portal have access to information such as HRA results, PHR data, and lab test results, and can send secure messages.
